Ulnar Deviation Strengthening
Why Include This
Ulnar deviation is produced primarily by a force couple between the flexor carpi ulnaris (palm side) and extensor carpi ulnaris (back side) pulling the wrist toward the little finger. These muscles are also key dynamic stabilizers of the ulnar wrist and the distal radioulnar joint, so building capacity here is commonly used in rehab for TFCC irritation, ulnar-sided wrist pain, and general wrist/grip conditioning. Direct high-quality trial evidence for isolated ulnar deviation strengthening as a standalone intervention is limited; the strongest support is anatomical and biomechanical, plus its inclusion in broader wrist-strengthening and grip programs. It is best viewed as a low-risk accessory movement grounded in muscle function rather than a treatment with its own robust outcome data. Progressive resistance and specificity principles (train the motion you want to strengthen) reasonably apply. Load should stay light because the moment arm is short and the deviator muscles are small.

Positions & Range of Motion

- Seated with the forearm resting on a table, palm facing inward and thumb pointing up (forearm in neutral), hand and wrist hanging just past the table edge holding a light weight. The wrist begins in a neutral, level position aligned with the forearm.
- End
- The hand has tilted downward toward the floor into ulnar deviation (little-finger side moving toward the wrist), forearm remaining flat and still on the table, then returns under control to the level starting position.
- Range of Motion
- Roughly 0 to 25-30 degrees of ulnar deviation, moving only within a comfortable, pain-free range; healthy wrists allow up to about 30-35 degrees.
How to Perform
A targeted wrist exercise that trains the muscles moving the hand toward the little-finger side (ulnar deviation). With the forearm supported and thumb pointing up, you hold a light weight and slowly tilt the hand downward toward the floor, then control it back up. It isolates a small, rarely-trained motion that stabilizes the wrist during gripping, carrying, and tool use.
