Cervical Protraction Awareness
Why Include This
Cervical protraction (the horizontal forward glide of the head, coupling lower-cervical flexion with upper-cervical extension) is a movement most people perform unconsciously for hours at a desk or phone but rarely train or even notice. This drill deliberately takes the neck into and out of that pattern to build joint-position sense, so the person can recognize a forward-head drift and reverse it. Evidence strength is limited and largely indirect. There is moderate-quality evidence that craniocervical flexion and cervical motor-control training reduce pain and improve deep-flexor performance in people with neck pain, and separate evidence that cervicocephalic joint-position-sense error is elevated in chronic neck pain and can improve with proprioceptive retraining. This specific "explore protraction, then return to neutral" awareness format is a reasonable clinical extension of those principles rather than a directly trialed protocol, so it is best classified as motor-learning/expert-opinion supported. Positions & Range of Motion

- Seated tall on a firm chair with feet flat on the floor, hands resting on the thighs, and the spine in a relaxed neutral. Eyes are level and looking straight ahead, with the ears stacked roughly over the shoulders (neutral cervical position).
- End
- The head has glided horizontally forward into a controlled protracted position (face and jaw translate anteriorly, back of the neck shortens) with eyes still level, then returns to the neutral stacked position. The endpoint of each repetition is a mindful return to neutral, aware of the difference between the two.
- Range of Motion
- Small-to-moderate horizontal translation of the head, roughly 2-4 cm from neutral to a comfortable end-range protraction, emphasizing pure forward/backward glide rather than chin elevation or depression. Kept pain-free and well within available range.
How to Perform
A slow, controlled motor-control drill that teaches you to feel and steer your head between a forward "protracted" glide and a neutral, stacked position. You glide the head horizontally forward, pause to notice the sensation, then guide it back to where the ears stack over the shoulders. The goal is not strength but proprioceptive awareness: learning to detect when the head has drifted forward so you can self-correct throughout the day.

Evidence-Based Dosing & EMG Data
2-3 sets of 8-10 slow, controlled repetitions, holding 2-3 seconds at the forward and neutral positions, once or twice daily. Movement should be smooth and pain-free; stop short of any range that provokes symptoms.

By movement pattern, sternocleidomastoid and scalene activity accompany the forward glide while the deep cervical flexors (longus colli/capitis) go relatively quiet, which is the functional rationale for using the return-to-neutral phase to re-cue deep-flexor control.

Supine supported glide
Lie on your back with the head resting on a firm surface or thin pillow. With gravity removed and the head supported, gently glide the head forward and back a small amount. This makes the horizontal motion easier to sense without having to hold the head up.
Fingertip-tracked mini glide
Rest two fingertips lightly on the chin and move through only a very small, pain-free range. The tactile feedback helps you confirm the motion is a horizontal translation, not a chin lift, and keeps the range comfortable for beginners.

Protraction-to-retraction sweep
Extend the movement to the full comfortable range in both directions: glide forward into protraction, then draw the head back into a gentle chin tuck (retraction), pausing 2-3 seconds at each end. This sharpens the contrast between the two patterns and adds light deep-flexor activation.
Task-integrated posture awareness
Practice the same neutral, stacked position while standing or during a real task such as reading, typing, or scrolling on a phone. Periodically check in, notice any forward drift, and reset. This transfers the awareness into daily life.
