Dart Thrower's Motion
Why Include This
The dart-thrower's motion (DTM) is one of the best-described planes in wrist biomechanics. Kinematic and cadaveric studies (e.g., work by Moritomo, Crisco, Wolfe and colleagues) show it is the functional plane the wrist uses for most power and precision tasks, and that during DTM the proximal carpal row (scaphoid and lunate) moves comparatively little while most motion occurs at the midcarpal joint. Because of this, DTM places relatively low strain across the scapholunate interval, which is the rationale for its wide use as an early, protected rehabilitation motion after scapholunate ligament injury, distal radius fracture, and scaphoid fracture/repair. The biomechanical evidence for the movement itself is strong; the evidence that DTM-based rehab improves clinical outcomes versus standard cardinal-plane exercise is still limited and largely drawn from small studies and expert consensus, so it is best framed as a biomechanically-grounded, well-reasoned choice rather than a proven superior protocol. For the general population it is valuable simply as a commonly untrained, real-world plane of wrist motion that builds coordinated control most people never practice in isolation.

Positions & Range of Motion

- Seated with the forearm resting on a table, elbow bent about 90 degrees and the forearm in a neutral (thumb-up) or lightly supinated position so the wrist is free past the table edge. Begin with the wrist "cocked back": radially deviated and slightly extended, so the thumb side lifts up and back, as if drawing a hammer back before a strike.

- The wrist moves smoothly along the diagonal into ulnar deviation with slight flexion, dropping the little-finger side of the hand down and inward, as if swinging the hammer down to strike a nail. The forearm has not moved; only the wrist has traveled through the arc. Reverse slowly to return to the start.
- Range of Motion
- Oblique plane roughly 45 degrees to the sagittal and frontal planes: from about 20-30 degrees of radial deviation with slight extension to about 20-30 degrees of ulnar deviation with slight flexion. Stay within a comfortable, pain-free arc; the full available range is normal to feel small at first.
How to Perform
A controlled wrist movement along the natural diagonal (oblique) plane the wrist prefers for everyday tasks. The hand travels from a "cocked-back" position of radial deviation with slight extension (thumb up and back) to ulnar deviation with slight flexion (little-finger side down and in), tracing the arc used when throwing a dart, swinging a hammer, or pouring from a jug. The forearm stays still while all motion happens at the wrist.

Think of drawing a hammer back and up, then swinging it down and in to tap a nail. Your hand follows that same diagonal both ways.

Active-assisted DTM
Use the opposite hand to gently guide the wrist through the diagonal arc, sharing the effort. Useful early after injury or when the plane feels unfamiliar and hard to isolate.
Gravity-minimized short arc
Rest the forearm palm-down flat on the table and glide only through a small, comfortable portion of the diagonal, reducing the extension/flexion demand. Grow the range as coordination and comfort improve.
Harder Variations ↑
Loaded DTM (hammer or light dumbbell)
Hold a light hammer or 1-2 lb dumbbell so its weight adds resistance to the arc, biasing the radial-extension and ulnar-flexion moments. Increase load gradually while keeping the forearm still and the motion clean.
Speed and power DTM
Once the range is well controlled and pain-free, progress to faster, more forceful repetitions to build power for sport and daily impact tasks (throwing, racquet strokes, hammering). Emphasize crisp control at both ends of the arc.
