Why Include This

Radial deviation is produced by the radial wrist muscles co-contracting: the flexor carpi radialis and extensor carpi radialis longus pull the hand thumb-ward while their opposing flexion/extension moments largely cancel, with contributions from the thumb's radial abductors/extensors (abductor pollicis longus, extensor pollicis brevis/longus). Strengthening it is grounded primarily in wrist anatomy and long-standing clinical practice rather than a large body of trial evidence — direct randomized trials on isolated radial deviation strengthening in healthy people are sparse, so the evidence is best described as limited/mechanistic. It is a reasonable, low-risk addition for building balanced wrist strength, addressing a commonly under-trained direction, and supporting demands like racket/club sports and tool use. One honest caution: because radial deviation loads the first dorsal compartment tendons (APL/EPB), it should be introduced gently and is often modified or deferred in people with De Quervain's-type radial wrist pain.

Positions & Range of Motion

Start
Seated with the forearm supported on a table or thigh, wrist and hand hanging just past the edge, forearm in neutral rotation so the thumb points up. Hold a light dumbbell or hammer with a relaxed grip; the hand starts deviated slightly toward the ulnar (little-finger) side.
End
The hand has tilted upward toward the thumb side (radial deviation), forearm still resting and unmoved, weight raised at the top of a short controlled arc before slowly returning to the start.
Range of Motion
Small active range — roughly 15-20 degrees of radial deviation, which is near the anatomical limit for this direction. Move only as far as is smooth and pain-free rather than forcing end range.

How to Perform

A targeted wrist exercise that trains radial deviation — tilting the hand toward the thumb side. Sitting with the forearm supported and the thumb pointing up, you hold a light weight (or a hammer gripped near its head) and raise the hand upward through a small arc, then lower it slowly. It isolates the radial wrist muscles that are rarely trained directly, building strength and control at a joint most people only ever move incidentally.

Recommended Dosing

2-3 sets of 12-15 slow, controlled reps per wrist, 2-3 times per week. Start with a very light load (1-3 lb or a light band) and progress load only when the full range is smooth and pain-free. Rest ~60 seconds between sets.

Press the thumb side of a loose fist gently up into your other hand or a fixed surface and hold 5-10 seconds, building strength with no joint movement — useful if motion is irritating.
