Deep Neck Extensor Endurance
Why Include This
The deep cervical extensors (semispinalis cervicis and cervical multifidus) attach segmentally and act as postural stabilizers of the cervical spine; they show altered activation, fatty infiltration, and reduced endurance in people with chronic neck pain, whiplash-associated disorders, and cervicogenic headache. Deep flexor training (craniocervical flexion) is well established, but the extensors are frequently neglected. Work by Schomacher and Falla and colleagues demonstrated that performing cervical extension with the craniocervical region held in a neutral-to-slightly-flexed position preferentially recruits semispinalis cervicis relative to the superficial semispinalis/splenius capitis, providing a rationale for this specific setup. How to Perform

On hands and knees, stack shoulders over wrists and hips over knees; let your head hang so your gaze is toward the floor and the back of your neck feels long.

First make a subtle nod, as if tucking your chin a fingertip's width — keep this nod the entire time so the small deep muscles at the base of your neck do the lifting, not the big ones up top.

Keeping the nod, slowly raise your head until the back of your neck lines up with your upper back — like lengthening the crown of your head forward and up, not cranking the head back.

In quadruped or prone-on-elbows, practice only the gentle craniocervical nod and hold the head level with the trunk (no lift into extension), building the endurance and control needed before adding range.
Reduced-range, shorter holds
Extend through only a small portion of available range and hold 3-5 seconds for fewer repetitions, progressing hold time and range as control and endurance improve.
