Why Include This

Deficits in cervical extensor strength and endurance are common in people with chronic neck pain, cervicogenic headache, and forward-head postures, and the deep extensors (semispinalis cervicis and multifidus) are the primary segmental stabilizers of the cervical spine. Prone extension against gravity is a simple, equipment-light way to progressively load these muscles through range. Evidence for cervical extensor and general neck-strengthening programs in chronic neck pain is moderate—systematic reviews support strengthening/endurance exercise for reducing pain and disability—but head-to-head evidence isolating this specific prone variant is limited, so it is best viewed as one reasonable loading option within a broader neck program rather than a uniquely validated technique. A neutral (chin-tucked) starting position is preferred to bias the deep extensors and avoid dumping the load into upper-cervical hyperextension.

Positions & Range of Motion

Start
Lying prone on a mat or treatment table with the forehead resting down (or the head just off the edge of the table), arms relaxed at the sides or hands under the forehead, and the cervical spine in a neutral, gently chin-tucked position.
End
Head and neck lifted into cervical extension so the face is approximately parallel to the floor, maintaining a long neck and an even extension curve, shoulders relaxed and down, before returning under control to the start.
Range of Motion
Cervical extension from neutral through mid-range; typically 0-30 degrees of controlled extension, avoiding end-range upper-cervical hyperextension or any pinching at the skull base.

How to Perform

A prone neck-strengthening exercise in which the client lies face-down with the head off the edge of a table (or over a rolled towel) and lifts the head against gravity into cervical extension, then lowers under control. It loads the posterior cervical extensor chain—especially the deep semispinal and splenius group—through gravity-resisted range, building endurance and strength of the cervical extensors.

Coaching Cues

    Begin with a gentle chin nod (tuck) to lengthen the back of the neck, then lift the head so the neck extends evenly through the whole length rather than cranking the top of the neck backward.

    Lift the head as if pointing the crown gently forward and up—leading with a long neck—until your face is roughly parallel with the floor; stop before you feel pinching at the base of the skull.

Recommended Dosing

Begin with 1-2 sets of 8-10 slow repetitions or 5-10 second isometric holds, 3-4 days per week. Progress toward endurance emphasis (2-3 sets of 10-15 controlled reps or longer holds) before adding external load. Work in a pain-free to mildly-fatiguing range and avoid provoking headache or dizziness.

Rest the forehead on stacked hands or a folded towel and lift only a small amount, holding an isometric contraction for 5-10 seconds. Reduces the gravitational lever and range for deconditioned or painful necks.

Keep the forehead lightly resting on the table and perform only the deep-extensor-biasing chin nod/retraction, activating the target muscles isometrically before adding a head lift against gravity.

Position the head and neck beyond the edge of a treatment table so the movement spans a fuller extension-to-flexion arc through gravity, increasing range and eccentric demand.

Add light resistance via a weighted head strap, light plate on the back of the head, or a manual/theraband resistance to increase strengthening stimulus once endurance in range is established.
