Why Include This
The main indication is restoring ankle dorsiflexion range of motion in patients with tight calves — a common finding in plantar heel pain, Achilles tendinopathy, mid-portion Achilles complaints, and post-immobilization stiffness, and a contributor to compensatory gait and forefoot overload. Static calf stretching has moderate evidence for producing short-term gains in dorsiflexion ROM, and gastrocnemius/plantar-fascia stretching programs have moderate RCT support for reducing pain in plantar fasciitis (e.g., first-step pain). Stand on a stair or step with the balls of both feet on the edge and heels hanging off the back. Hold the railing or wall for balance and let your heels sink below the step level under body weight to stretch the calf. Keep the back knee straight to bias the gastrocnemius, then bend it slightly to target the deeper soleus.

Evidence-Based Dosing & EMG Data
Hold each stretch 30 seconds, 3-4 repetitions per leg, doing both a knee-straight (gastrocnemius) and a slightly knee-bent (soleus) hold. Perform once or twice daily; accumulating roughly 2-3 minutes per calf per session is a reasonable target. Stretch to gentle tension, never sharp pain, and ease off if there is nerve tingling or numbness in the foot.
This is a passive/gravity-driven static stretch, so target muscle EMG activity should stay low; the posterior calf (gastrocnemius, soleus) is under passive tension rather than active contraction. Excessive gripping or high plantarflexor EMG during the hold indicates the patient is guarding and should be cued to relax and let body weight lower the heel.

Floor / wall calf stretch (heel down)
Perform a standard standing calf stretch with the heel flat on the floor and hands on a wall, staying off the step edge. Removes the balance and drop-below-neutral demand while still lengthening the calf.
Reduced-depth step stretch
Lower the heels only partway (to level with the step) instead of a full drop, and use two hands on the rail. Cuts the dorsiflexion load for sensitive ankles or early-stage Achilles pain.

Single-leg step calf stretch
Perform the drop on one foot at a time, letting more body weight load a single calf for a deeper stretch and added balance challenge.
Eccentric heel-drop transition
