Heel-Toe Walking / Tandem Gait
Positions & Range of Motion
- Start
- standing tall at one end of a straight line/taped seam, feet together, arms relaxed at sides or slightly abducted for balance
- End
- advanced several steps forward in tandem, heel of the front foot placed directly in contact with the toes of the trailing foot, on a very narrow base
- Range of Motion
- whole-body forward locomotion along a straight line, each step ~one foot-length; individual joints move little — emphasis is on progressively narrowing the base of support
How to Perform
Dynamic balance exercise where patient walks in a straight line placing the heel of one foot directly in front of the toes of the other foot. Challenges balance, proprioception, and vestibular integration. Commonly used for fall prevention and gait training in older adults.

Dynamic activation of ankle dorsiflexors and plantarflexors for balance corrections. Tibialis anterior and gastrocnemius/soleus provide continuous postural adjustments during narrow-base walking.

Walk with a small gap between heel and toes instead of touching. Slightly wider base of support.
Wall-Assisted Tandem Walk
Walk tandem alongside a wall, fingertips touching the wall for safety. Reduces fall risk during practice.

Eyes-Closed Tandem Walk
Perform tandem walking with eyes closed. Eliminates visual input, increasing vestibular and proprioceptive demand.
Tandem Walk with Head Turns
Walk tandem while turning the head left and right. Adds vestibular challenge (VOR demand).

Tandem gait is a standard clinical balance assessment and training tool. Challenges narrow base of support, vestibular processing, and proprioceptive feedback. Evidence-based fall prevention component. AUDIT[low]: evidenceLevel is 'strong' but sources are only generic textbooks; the supporting fall-risk reference (Wolfson 1996) appears only as a cue citation, not in sources. → Add a primary balance/fall-prevention source to the sources array or downgrade the evidence level.
