Why Include This

This is a functional loaded-carry (gait-under-load) task that trains the whole postural chain to resist fatigue while walking — directly transferable to daily activities like carrying groceries, a child, or luggage. The mechanistic and clinical basis is well established even where carry-specific trials are limited: (1) added axial/weighted-walking load is an osteogenic stimulus, and weighted-vest walking programs have shown benefit for bone-mineral density and postural strength in older and postmenopausal adults, so a graded weighted backpack is a low-cost home proxy; (2) walking with an anteriorly-compensated posterior load imposes sustained isometric demand on the spinal erectors, quadratus lumborum, and deep abdominal stabilizers, building the trunk-endurance that is repeatedly associated with reduced low-back-pain risk (McGill's endurance work); and (3) gait under load recruits the hip extensors and abductors for single-leg-stance control, supporting balance and lower-limb power relevant to fall prevention. It is scalable simply by changing pack weight, distance, or pace, which makes it practical for progressive home dosing. Evidence for the general strategy is moderate; evidence for the backpack specifically is more limited and mechanistic, so it is rated limited here.

How to Perform

Load a sturdy backpack with books or water bottles, wear it snug on both shoulders with the load high and close to the spine, brace the trunk, and walk at a steady controlled pace for a set distance or time. The backpack is the only equipment needed, making this an accessible loaded-carry / "rucking" drill at home.
