Positions & Range of Motion

supine; for the curl-up one knee bent with hands under the lumbar spine, for the dead bug arms and legs raised to 90° with the low back pressed to the floor
End
curl-up: head and shoulders lifted minimally with the lumbar curve held; dead bug: opposite arm and leg lowered toward the floor while lumbar contact and quiet ribs are maintained, then returned
Range of Motion
minimal spinal flexion in the curl-up; dead bug ranges through near-full opposite hip/shoulder extension while keeping the spine still (anti-extension control)

How to Perform

Two complementary anterior core exercises. Modified curl-up: supine with one knee bent, hands under lumbar spine, lift head and shoulders minimally. Dead bug: supine with arms and legs at 90°, lower opposite arm and leg while maintaining lumbar contact. Effectively activate rectus abdominis (50% nEMG) and external obliques while minimizing lumbar compression. Safe and well-tolerated in patients with low back pain.

Research Notes

Modified curl-up: rectus abdominis 50% nEMG (28-65%) [Calatayud 2019]. Dead bug: peak RA and EO amplitudes increase significantly with intensity (p=0.008 and p<0.001) [Kim 2016]. 5 graded intensity levels available [Kim 2016]. Plank with brace: EO 77% nEMG [Calatayud 2019]. Plank with hollowing significantly increases all oblique activation (IO: ES=2.0-2.3, EO: ES=1.0-1.4, lumbar ES: ES=0.6) [García-Jaén 2020]. Well-tolerated in chronic LBP; lateral plank mostly non-tolerated [Calatayud 2019]. AUDIT[medium]: Cue 'Press the lower back into the floor the entire time' contradicts the modified curl-up mechanics described (hands under lumbar spine to preserve neutral lordosis). McGill's modified curl-up deliberately maintains the lumbar arch rather than flattening it. → Scope the flatten-the-back cue to the dead-bug component only, and for the curl-up cue maintaining gentle constant pressure on the hand under the lumbar spine (neutral lordosis). AUDIT[low]: Iliopsoas note says 'moderate demand beyond pure stabilization [Juker 1998]'; Juker 1998's finding was that psoas activity stays low during curl-ups (a reason curl-ups are preferred), with higher psoas demand in straight-leg/sit-up tasks. The characterization risks overstating psoas demand for the curl-up component. → Qualify the note to the dead-bug leg-lowering phase, where hip flexion loads iliopsoas, and reflect that the curl-up keeps psoas activation low per Juker 1998. AUDIT[low]: The sources list includes beales-slr-motor-control-2009 (active straight-leg-raise motor control) and marcolin-pushup-variants-2015 (push-up EMG), neither of which is relevant to the curl-up / dead-bug content — these appear to be mis-linked citations. → Remove or replace the unrelated SLR and push-up citations with sources that actually support the curl-up / dead-bug EMG claims (Calatayud 2019, Kim 2016, Juker 1998 are already linked).
